Forma is operated by PolyphasicDevs ("we," "us," or "our"). This Privacy Policy explains how Forma handles information when you use the Forma mobile application.

Information We Collect

Apple Health Data

If you grant Forma permission through HealthKit, the app may read health and fitness data such as:

• Steps
• Walking and running distance
• Active energy
• Heart rate and resting heart rate
• Walking heart rate average
• Oxygen saturation
• Respiratory rate
• Body mass and height
• Sleep analysis
• Mindful session data
• Workout data

Forma uses this data to provide analytics, trends, records, goals, insights, and exports within the app.

App Data Stored On Your Device

Forma may store the following on your device:

• Your selected export folder bookmark and display name
• The date of your last export
• Whether you completed HealthKit authorization
• Personal records
• Goals and related app preferences
• Local UI preferences and dismissed notices

Purchase and Subscription Information

If you buy Forma Premium, Apple processes your payment. We do not receive your full payment card number or full billing details. We may receive subscription status information needed to determine whether premium features should be unlocked in the app.

How We Use Information

We use information to:

• Request and maintain HealthKit access you authorize
• Generate analytics, summaries, trends, and in-app insights
• Save your preferences and locally stored records and goals
• Export your data to a location you choose
• Determine whether you have an active premium entitlement
• Restore purchases when you request it
• Diagnose app errors and improve reliability

How Data Is Shared

We do not sell your personal information.

We do not share your HealthKit data with third parties for advertising, marketing, or data-brokering purposes.

Information may be handled by third parties only where necessary to operate app platform features you choose to use, such as:

• Apple HealthKit, to provide health data access you authorize
• Apple App Store and StoreKit, to process subscriptions, purchases, and restorations
• A storage provider or file location you choose when exporting data

Health Data and HealthKit

Health data accessed through Apple Health is used only to provide health and fitness features in Forma. We do not use HealthKit data for advertising, marketing, or any purpose unrelated to providing health and fitness functionality.

You can revoke HealthKit permissions at any time in the Apple Health app or iOS Settings.

Exports

When you choose to export your data, Forma writes export files to the folder or location you select. Once exported, those files are subject to the privacy and security of the storage location you chose and any services you use to manage or sync that location.

Data Retention

Forma generally retains app data on your device until you delete it, remove the app, revoke permissions, or overwrite that data through normal app use.

Exported files remain where you save them until you delete or move them.

Subscription records may continue to be available through Apple based on Apple’s policies.

Your Choices

You can:

• Decline or revoke HealthKit access
• Delete the app from your device
• Delete exported files from your chosen storage location
• Manage or cancel subscriptions through your Apple account settings
• Restore purchases through the app when applicable

Children

Forma is not directed to children under 13, and we do not knowingly collect personal information directly from children through an account system or web form. If you believe a child has provided us information directly, contact us and we will review the request.

Security

We use reasonable measures appropriate to the app’s design to protect information handled by Forma. No method of electronic storage or transmission is completely secure, and we cannot guarantee absolute security.

International Use

If you use Forma outside your home country, you understand that information may be processed by Apple or other platform providers according to their own systems and policies.

Changes to This Policy

We may update this Privacy Policy from time to time. If we make material changes, we will update the effective date above and, where appropriate, provide additional notice.
